If a player has suffered a permanent disabling injury or illness occurring after year-end ratings were achieved (with supporting documentation), a medical appeal may be submitted.

A $100 filing fee will be required for all Medical Appeal submissions. The filing fee will only be
returned to the filing party if the medical appeal is submitted to the USTA National Medical

Medical appeals must be submitted separately in writing to the Director of League tennis, Lindsey Dye Brashier, at lindseyb@mstennis.com. In addition to the forms below, a written request must also be submitted. This request may be made via email.
